From the company’s inception in 1996, our mission has been to help people in New Jersey live safely at home as long as possible while maintaining their independence and dignity. The safety and well-being of the clients we serve is most important to our Health Force team of dedicated health care professionals.
Utilizing a team approach, Health Force, a New Jersey licensed health care service firm, achieves this goal by integrating the services of community programs, physicians, case managers and discharge planners. Along with the coordination of services provided by our nurses, schedulers and home health aides, we are able to meet the needs of the patients and families we serve.
Many Health Force home health aides have had consistent work history with us. The advantage of maintaining continuity of aide placement gives our aides the ability to be acutely aware of small changes in a clients’ physical or mental condition. Early intervention at home may avoid a hospital stay. Additionally, our employees adhere to a strict 12- hour call out policy giving us the time needed to find a replacement. We make every effort to find a replacement aide when call outs occur and keep you informed of our progress.

Accreditation
Accreditation of health care service firms in New Jersey provides agency oversight and accountability for the public who utilize their services. 900 home care providers are licensed in New Jersey as health care service firms. Approximately 175 are accredited by the Commission on Accreditation for Home Care (www.cahcnj.org). The majority of the remaining providers operate without an accreditation
Health Force voluntarily submits to a rigorous annual on-site monitoring visit by a registered nurse (RN) surveyor from the Commission on Accreditation for Home Care (CAHC). This monitoring visit ensures the public that our agency complies with all standards set forth by the New Jersey Board of Nursing, the New Jersey Division of Consumer Affairs licensing division, and the New Jersey Medicaid program.
The Health Force Trenton office has been accredited by CAHC since 1996 for Personal Care Services and has maintained the highest designation of “Full Accreditation with Distinction” for more than a decade. This elite designation is your assurance that Health Force continues to deliver the highest level of quality patient care.
